New Legal requirements coming into effect on June 17, 2017 require us to make changes to our F51-208A form (Confirmation of Identity).

The old version of the f51-208A form, found at the end of the F17A application (paper version), will no longer be accepted as of June 17, 2017.

Please note that the new form will be available to download from the Document Centre in the Advisor Centre.  Please ensure this version accompanies each non-registered application going forward.
